Welcome! Thank you for choosing New River Valley Medicine for your health care needs. In an effort to provide our community with support for men’s hormonal health, we are offering a dedicated and structured program to address issues related to low testosterone in the male patient.
Below are policies that we request participants read and sign to participate in this program. We strive to provide patients with the highest level of customer service. Our entire team appreciates and welcomes your feedback to improve services, and to address any personal concerns regarding your medical care or office experience.
Consultation Fee:
A consultation fee of $25 will be charged for the initial patient visit. During your initial consultation, your provider will discuss your symptoms with you and may recommend lab work to investigate the underlying issues. They will consider your symptoms and the results of your lab work in order to formulate a personalized treatment method that will be the most successful for you.
Self-pay Only Program Fee:
Most insurance companies that approve testosterone replacement therapy have copays higher than what we charge in our clinic. We are able to charge a lower amount due to higher volume of medication use within the clinic. We are also accepting patients into many of our programs who have a primary care provider elsewhere. Therefore, we have built payment options with our patients’ needs in mind, as well as simplicity for our staff who oversee our programs.
Though we will not be billing insurance, you are welcome to file a claim directly with your insurance and see if they will reimburse you. This all depends on how your specific policy is set up, as to whether they will pay you back.
Lab work:
Periodic lab work is required for initial dosing and measuring response to therapy. The cost of this lab work is not included in consultation or program charges. Patients will be provided with an order for this lab testing which can be performed at any lab and can be billed to insurance, if applicable, per your agreement with that lab.
Medication:
As ordered by the provider, weekly testosterone injections, included in the program fee, will be administered in our office. Medication dosage is determined by the medical provider based on individual patient needs, and is subject to change at the provider’s discretion.
We are now offering testosterone pellets as an option for patients who qualify. Price will vary depending on how many pellets each patient requires.
Appointment Schedule:
Patients are scheduled with a medical provider once every 3 months. Providers for these face-to-face encounters may vary based on appointment schedules and availability. Patients will have weekly visits with our nursing team for injection administration, vital signs review, and monitoring of response to therapy.
Withdrawal and Program Termination:
Patients may withdraw from the program at any time, but no refunds will be provided. A 7-day notice is required to stop the next scheduled payment. Patients may be removed from the program without advance notice at any time due to non-compliance, breach of this agreement, patient health status changes, or unacceptable behavior.
Program Inclusions:
The program includes provider visits, medication, administration supplies, and qualified personnel to perform injections.
